For illness, please ensure you have also telephoned Mr Rees (Upper School) or Mrs Gemmell (Lower School) after 6.30am IN ADDITION to completing this form.
Date of absence
If requesting an absence in the future, please complete the box above and Mr Rees or Ms Starkey will come back to you.
Include any duties, trips or other notes that are not on your timetable. Email lessons details to staffcover@hornsbyhouse.org.uk
